  • Ivabradine is a novel selective cardiac medication that works by inhibiting the cardiac myocardial hyperpolarization current (If). This current inhibition decreases the efflux of sodium ions from the cardiac sarcolemmal sodium-potassium ATPase pump, resulting in decreased contractility and a decreased heart rate. This mechanism of action differs from that of beta-blockers, and it avoids some of the side effects associated with beta-blockers, such as increased airway resistance and arterial blood pressure.

    • Management of chronic stable angina pectoris in patients with normal sinus rhythm
    • Treatment of heart failure with reduced ejection fraction
    • Used to reduce the risk of hospitalization for worsening heart failure
    • Effective in patients who cannot tolerate beta-blockers or in addition to beta-blocker therapy
